Birth/Death/Marriage Certificates
Information and/or copies of birth, death or marriage certificates may only be obtained from the Registrar General’s Office which is a department of the Provincial Ministry of Consumer and Business Services. Applications for birth, death and marriage certificates are available at the City Clerk's Office, City Hall. The application is mailed, with the required fee, to the Office of the Registrar General in Thunder Bay. Alternatively, you may wish to download the document directly from the Registrar General’s web site through “www.cbs.gov.on.ca”. The Registrar General’s Office currently only provides same day service from their office at Queen’s Park in Toronto.
