Where a claim is to be filed with the City, the Incident Report Form found below is to be completed.  The form should be submitted to the Risk Management Department as soon as possible following the incident.  Upon receipt, the Risk Management Department may forward the form to its insurance adjuster for an investigation. 

Citizens are advised to contact their own insurance company at the time of an incident.  If the insurer believes the City is liable they will seek compensation on your behalf.
